- Your full name exactly as printed on your RGUHS nursing certificate
- RGUHS registration or roll number and the name of your nursing college
- Programme and year of passing (for example GNM 2017, B.Sc Nursing 2020, M.Sc Nursing 2022)
- Copies of all mark sheets and your degree, diploma or provisional certificate
- State nursing council registration details, where relevant to your target agency
